COVID Employee Reporting Form

This form is to be completed by any Lawrence County Schools Employee reporting a COVID Close Contact to a person that has tested positive for COVID, is COVID Symptomatic, or has tested positive for COVID.

If you are completing this form before you have received your  COVID test results please complete the form anyway.  Upon receiving your COVID test result please edit your form via the email that is sent to you following the initial completion of the form.
